DOCTOR'S QUESTIONS

When you contact the office on behalf of your sick child the doctor will want you to answer questions like:

1. What is his/her temperature.

2. How is his/her appetite?

3. Any vomiting or diarrhea?

4. Any discharge from the ears, nose or eyes?

5. Any cough? Wet or dry? How often?

6. Sleep disturbances?

7. Abdominal (belly) pain?

8. Rash?

9. Immunization status?
